Protected Health Information
The Health Insurance Portability and Accountability Act (HIPAA) requires the San Bernardino County Department of Public Health (DPH) to protect health information that we may have about you, and we take the privacy and security of your information very seriously.
When you receive certain health services from DPH, we will provide you with a Notice of Privacy Practices that informs you of your rights and our responsibilities regarding Protected Health Information, its uses and disclosures.
