Confidential Morbidity Reports & Reporting Forms

The Communicable Disease Section (CDS) conducts surveillance and response activities, investigates cases and outbreaks of communicable diseases including gonorrhea, syphilis, and HIV/AIDS, provides educational activities designed to reduce or control the incidence of infectious diseases, and implements interventions to respond to and mitigate disease cases and outbreaks in San Bernardino County.

The Communicable Disease Section relies on our public health partners to report more than 85 communicable diseases as mandated by California law. California Department of Public Health (CDPH) have recently updated Title 17 sections 2500 and 2505 of the California Code of Regulations. 

Notable Changes Include (as of October 2026):
The following conditions have been newly added, reclassified, or renamed:

A) Conditions Now Requiring Immediate Reporting by Telephone 

  • Melioidosis has been newly added as a reportable condition and must be reported immediately by telephone. 
  • Monkeypox/Orthopoxvirus infection has been reclassified from one working day to immediate reporting.
  • Neisseria meningitidis (invasive disease) now must be reported immediately by telephone.

B) Conditions Now Reportable Within One Working Day

  • Brucellosis, human has been reclassified from immediate to within one working day.
  • Cronobacter sakazakii infections in infants less than one year of age have been newly added.
  • Cyclosporiasis has been reclassified from a 7-day timeframe to within one working day. 
  • Hemolytic Uremic Syndrome (HUS) is now reportable within one working day, reclassified from immediate notification. 
  • Legionellosis has been reclassified from a 7-day reporting requirement to within one working day to enhance outbreak response capability.
  • Multisystem Inflammatory Syndrome in Children (MIS-C) is a newly added condition now reportable within one working day.
  • Shiga toxin detected in feces is now reportable within one working day, reclassified from immediate reporting.

C) Condition Now Reportable Within One Calendar Day 

  • Coronavirus Disease 2019 (COVID-19) is now reportable only when hospitalization occurs. 

D) Condition Now Reportable Within Seven Calendar Days

The Confidential Morbidity Report (CMR) is required to be completed by providers. On the reverse of the CMR is a list of conditions reportable by providers under California Code of Regulations Title 17 Section 2500.

Confidential Morbidity Reports (CMR)

Respiratory Syncytial Virus (RSV) has been reportable in San Bernardino County since 2002 for cases of all ages as a locally reportable condition. In 2016-year, RSV deaths in persons under 5 became reportable in California. State reporting requirements do not supersede what the local health officer has in place. Providers still need to report all cases, not just deaths for anyone with RSV in San Bernardino County.

Forms By Disease

Sexually Transmitted Infections:
Use the Confidential Morbidity Report Form (CDPH 110 A) – All Conditions Except HIV/AIDS, TB, and to DMV to report STDs.

HIV:
HIV/AIDS can be reported by providers by completing the HIV/AIDS Adult Confidential Case Report Form. This form can be faxed by provider offices to CDS at (909) 381-8474 or mailed via traceable mail.

HIV/AIDS laboratory reports including positive antibodies, viral loads, and CD4 counts can be mailed by traceable mail to CDS. Faxing by laboratories of HIV laboratory reports to CDS is not permitted.
