ATTENTION – We issue certificates of births, deaths, and fetal deaths for events occurring in the last 24 months.

The Vital Statistics Registration Office (VSRO) is responsible for registering all births and deaths and issuing burial permits in San Bernardino County. Our office holds records for 24 months, all records older than 24 months must be purchased at the San Bernardino County Hall of Records.

Mountain View Avenue, San Bernardino, CA 92415.                                                                                                                                                                                Hours of operation are Monday – Friday, 8:00 AM – 5:00 PM.

Our Office will be closed June 20th, in observation of Juneteenth 

The Vital Statistics Registration Office Lobby  is open. No appointment is required.
